How a cost report at a geography treats higher-level (umbrella / national) line items: DIRECT counts only lines attributed to the geography's subtree; FULLY-LOADED adds an apportioned share (per capita, per dose, …), stated in method. The ledger is never allocated downward — apportionment is a report-level statement, traceable through evaluatedResource. Absent ⇒ direct (cost-v1).
